Form stopped populating submissions on Google Sheet ingration

Hello! One of my forms stopped populating my Google Sheet which was setup with integration. How can I fix this?

I checked the integration and it looks like the forms permission to access the sheet has been removed:

The integration does still appear to be active, but I am not sure the permission can be restored.  Removing the integration and adding it back again will create a new sheet with all current form data.
