Fields visible in build mode disappear in preview

I had a template form that was working okay after a few simple changes. But, after moving fields around on the form they are no longer visible when I go to preview it. What is causing this and how can I resolve it?

I can see that you have setup a condition to show and hide fields if condition is met. Also, some of the fields where already deleted. This is the reason why it is hidden.

Please fix the condition and if not needed, just completely remove it from your form.
