I have the odd client fill a form, then throw a weird error, then makes them fill the page again. Very frustrating to clients. Can you see why?
Also, when I get form submissions it also shows me an HTML version? WHY? see screen shot

I tried filling your form but I could not reproduce the problem. What specific error did they see?
I also made a test submission on the cloned version of your form and it went through.
With regards to the email, your email notification is also fine.

It's most likely an issue with your email client. It did not render the HTML properly. Please check your email client settings. You may find settings related to it.
