When selecting edit form, the screen just goes blank and never populates the screen for editing.

I have just checked the form you shared while logged into your account and it worked on my end, please see this screenshot:
I would recommend you to do the following:
1. Try clearing your browser's cache: https://kb.iu.edu/d/ahic
If clearing the cache does not work, then try this:
2. Use a different browser or internet connection, that will help to know if the problem is related to the browser or connection.
3. Try also using a different device and/or network.
