How to enable Text Field in email notifications

I am creating a credit card authorization form. I put in a box where the client will enter their first and last name, their last 4 digits on their card and the amount they authorize us to charge to this card. I then created a Text Box where the wording is auto filled with this information like this:

I, {name}, authorize {selectYour} to charge my card on file ending in {last4} for the amount of ${paymentAmount} to pay toward my vacation. I understand that this charge will go directly to the company that my vacation is booked through.

The problem is that this is not showing up in our email or our PDF forms for us to save in the client's file. We need this paragraph in the email and PDF. So, how do I make this happen? I can get it to show in the main window on the Jotform Submission page but I can't save this in any form that allows me to put it with my client's information in the system.

Also, I'm concerned with the ability to edit forms after the client has signed and submitted them. Is there a way to lock the forms once they have been submitted so that they cannot be edited?

    Thank you

Its is possible to display Text and headers on your email by following the guide below.

I have also note that your form is collecting Credit Card information without using the Payment Fields we have integrated with. Kindly note that this is a violation of our terms of use and will lead to the suspension of your account if yo do not use the Payment Fields we have integrated with.

Below is a guide on how you can create payment forms

On the issue of disabling users from editing their submissions, unfortunately, we don't have that function. If you like, you can manually remove the "Edit Link" in the email notifications.

Kindly do let us know if you need further assistance

    Thank you.

I will try this tomorrow but wanted to make sure that I changed the field to not ask for the last 4 digits. I realized that I could not collect a full credit card number due to PCI compliance but as it is legal to ask for the last 4 digits without being PCI compliant, I didn't realize I couldn't collect that either. Thank you for pointing that out.

In addition to what was suggested to you, to include the TEXT FIELDS on your email templates (both for NOTIFICATIONS and AUTORESPONDERS), you'd have to manually copy/paste them.

The article below should guide you how to edit your email templates.
