Embedded form suddenly disappeared from webpage.

    Hi there, hope all is well and thanks in advance for your help.

      We have been using JotForm to collect leads, and overall it's a good tool but we have had problems with it that have brought up concerns.


      First, we are unable to give our forms a name, without it showing up on the actual webpage. Meaning, if we embed a form, it has a title that shows up at the top of the form, and the only way to remove this is by having a form without a title. It makes this software very difficult to use. See screenshot here since it won't upload. How has this issue not been resolved yet?

      Last and more importantly, somehow an embedded JotForm just simply disappeared off a website that had advertisements running to it. This is a *huge* problem. The form was working completely fine and out of nowhere, it had just stopped displaying. How does this happen?

      The webpage itself had not been edited or altered in over 22 days, yet somehow 9 days ago the form completely stopped displaying. Hundreds of dollars were spent on advertising driving traffic to a page that did not have a form to fill out because the JotForm had totally disappeared. After checking the webpage, the form was indeed still there, and not one single thing had changed.


     How in the world did this happen, and why? If problems like this occur, there is no way that we can continue to use this product.

    On your first issue of form title:

    Please note that there are two title in form. One it "Title" which is a descriptive name of your form displayed in your account. Another is "Page Title" which is displayed as the title in the webpage. You can retain the "Title" of your form which will solve your problem. Please check the screenshot below:
