I can't see appointment time requests

    I have been following a thread on how to create appointment times and have been successful in creating the form but I cannot see which appointments have been chosen. Have I missed a step?

    I can fill in a submission with ticks against different times but when I submit, just the name shows up.

    Whenever an appointment is chosen, it will not longer be available in the form. You can also see it by checking your submissions page: https://www.jotform.com/help/269-How-to-View-Form-Submissions 

    As well, you can setup email notifications to see the data via email: https://www.jotform.com/help/25-Setting-up-Email-Notifications

    Thanks BDAVID,
    The problem is that I can’t see which appointments I have made for ‘my child’ rather than the appointment is no longer available. It also still shows those appointments as still available.
    Any ideas what I have done wrong?

    There was an issue with the Appointment Slots widget when you created your form.

    I have cloned your form and newly added widgets work fine but not the ones that were added before.

    I suggest you remove the widgets and add them again or create an entirely new form and everything should work correctly. Do not clone the form as it won't work.

    I also suggest you add an email field so the parents get an email with the selected appointments.
