complete print approval form with legal language

Hello,

 I do many custom projects. I was wondering if you had print approval forms for custom t shirts that I can show an image of the model and that the customer approves item completly as show, with all of the legal language on the same form that they sign?

It would be possible to create a form template and then use an image field in the form to insert the specific image for the current client.  You would only need to update the image used in the image field for each new client.
