The form you referenced is still active.  If you no longer have access to the account, you can clone a new copy of the form to your own account:

Thanks. I was able to clone the form but could not import the data already in. Please is there a way to import the data to avoid having to complete the already filled forms?
Regards and thank you for your valuable cooperation. Regards

The submissions will not be cloned when you clone a form. If you have submissions of the original form in Excel or CSV format, you can import the data with our import app - http://import.jotform.io/
