Can I Create a Financial Consent Form with Electronic Signature and Auto-Uploading Feature to Patient File?

Our medical clinic requires the capability to send a link to our patients with an online financial consent form template for them to complete prior to procedures. We require it to have their electronic signature, and then the ability to upload into their patient file. Is this something that can be done through Jotform?

You can also upload the signed form to a patient file, depending on where you want to upload it. You can take advantage of our integrations with Google Spreadsheets and the ability to export to PDF and CSV or Excel.
