Hi, My account adam619 has been suspended. Why is that happened? I didn't ask for any private information.

Your account is suspended because of the Copyright logo.
Copyright Infringement. Company respects the intellectual property rights of others. Accordingly, Company has a policy of disabling access to any Submission that violates copyright law, suspending access to the JotForm Service to any user who uses the JotForm Service in violation of copyright law, and/or terminating in appropriate circumstances the account of any user who uses the JotForm Service in violation of copyright law.
You have to agree to our terms and only then your account can be activated. The logo will also have to be deleted from the form.

Based on the answer, I have provided the proof for the logo. For your information, I am the employee of the company. Hence, I have the authority to use the logo. You can see the proof that I provided at the support team.

Thank you for providing a proof for the logo. I activated your account. I would suggest you to change the email address of your account and use your corporate email address.
Also, please note that your forms are still disabled because your account is overlimit. The Free account can receive up to 100 submissions per month and you received 140 submissions this month.
You can upgrade your account to get higher limits and to enable your forms immediately. You can find more about different account types and limits on our Pricing page.
However, if you don't want to upgrade, you have to wait until your submissions counter resets to zero which will be on the first day of the following month.
