Hello,
I need to make 2 forms that can be used on my squarespace website.
I need a form that customers and employees can upload photos and videos to in addition to basic fields like: address, subject and message.

Hello Skyler - Regardless of which platform you're using for your website, the first thing you need to do is build your form based on your needs. If you need something that asks for file uploads, address, subject, message, my recommendation is for you to fiddle around with our Squarespace Form Builder.
All the fields you need are available from the left hand side of the builder. On top of that, below are some detailed articled that will guide you along the way on how to build forms.

Once you're done building your form, it's time to embed it on your website. With Squarespace, we got you covered on this guide.

Yes, your users can upload that size. You can also change the File Size Limit on option tab of File Upload field.
