Could I change Contact us form in the form templates gallery

Hey there,

Are there any possible changes in Contact us form. I would like to add one more field - Rate the website and change the color. Is that possible?

In order to use and change this template, you should clone JotForm themes by clicking “Use Templates” button as seen below. Please follow the screenshots ın order to add a new field and change the color. 

Also, I recommend ünlemyou to use our  “Star Rating” field to give better presentation to form user while rating.
