New Layout: Why isn't the email autoresponder sending?

3. On the same form the default autoresponder is not firing. It was firing previously but has stopped and I can't figure out why.

I have been checking your form and you have a condition that triggers the email: 

I have tested the form and filled it based on the condition, I received the email without any problem, please note that the email will not be triggered if the condition is not met. 

Please test your form filling out the involved fields, the main email field as well as the last one, and the email should be sent.
