Emails: How to include the entire form

Headers, text, and images are not shown on the email body by default. If you want them to show in the email body you would need to add that information manually in your email settings. 

However, it is possible to include header, texts, and images in a PDF. And you can attach it to the email. To do that you need to enable show headers and text.
