Cannot find a form

Hi,

I had a colleague create a form and I cannot find the form. She must have clicked an old form and now I have people using the form and no idea where their information is going. I would love to get a call for help. 

I have no idea where the link below is going. 

The mentioned form belongs to the username bmccombs  (email account: webmaster@w####.edu)
