I am concerned because I have two refund charges from JotForm and today someone tried to submit a form on our site and she couldn't submit correctly. Can someone please contact me?

I have checked your account and see that it is currently holding Bronze yearly subscription. It looks like that you have updated the credit card information which has created a duplicate subscription. So, we have cancelled one of the subscriptions and processed a refund. I see that a note has already been sent to your email email@example.com by our colleague. The account is active and good in standing.
Please let us know the issue with the form so that we can take a look and assist you further. It would be helpful if you can provide us with the link to the web page where the form is embedded.
