Contact created through my form are not sending to Salesforce

The integration between Salesforce and Jotform is there, and I was getting records entering into the Salesforce from the Form, now it is not working...

I have tested the Salesforce integration with a test form and it seems to be working fine at my end. Could you try removing it and try reintegrating it to see if that works?
