Is there a field or widget that will allow my users to input multiple entries?

We're building a form for our ecommerce site where the customer can request to add users to our site.  We're trying this style:

But they may have more than one user they want added. I tried the input table but that doesn't allow me to have different field types. 

I'm trying a workflow so it asks if there are more employees to add but we felt like it is difficult to see who they added prior so the idea is to have it like the input table where they can add users in a table.

Any thoughts as to the proper widget to use for something like this?
