Hi,

Summer Camp Registration Form that can accept payments and tie into accepting my online waiver/policies. Which template allows me to do this? I need your service on a temporary basis, between now and July 15th. Would your free service allow this or do I need to purchase something on a monthly basis? I'd add your template to my website:

Or you can build from scratch in the form builder so you can create based on your requirements.

This is all possible with the free plan but with the free limits, you can only accept 1 payment submission per month. If you are expecting less than 10 payment submissions per month, you can upgrade to Bronze. If it's more than that but less than a hundred, you can upgrade to Silver. https://www.jotform.com/pricing

When you add payment integration in your form, the submission is called payment submission. Each successful payment processed through your form is counted as 1 payment submission.

Thank you for that info. I think I’ll go with the bronze plan and process 10 or less payments each month and upgrade as necessary. Are their any fees associated with processing client card payments or is this included in the $19 monthly charge? 

There's no other payment other than the subscription. JotForm does not add extra fees on payment submissions.

However, depending on the payment gateway you're using in your form, they may charge you an extra fee for every payment transactions. Example, if you are using Paypal, there may be a small fee for every payment you received.
