Why the background image is not showing?

I am creating a form on jotform and I want to use the Nurse theme that is available free under Themes. However, when I try to use the theme, the background does not appear. I want to use the nurse theme because I'm creating a form for a nursing event. What am I doing wrong?

I checked your form and the image file is missing:

The URL is broken http://www.somospodologos.es/sites/default/files/sugerencias.jpg

So that's why it is not showing.

Thank you for your response. I believe I did exactly what you stated. I edited the form and went to page image and entered the URL to the image. However, it's still not working. When I go to the URL by itself, it state URL not found. Is the image no longer available? Do you know if there are any other nursing themes/background. I really want a nursing theme since the form is being created for a nursing event.  Any suggestions?

Did you clone that form or used a template?

Since the website where the image was stored is down.

And it was some external website: http://www.somospodologos.es/

So we cannot do anything about that. The solution would be to find some other image and place it in the background of your form.

I cloned the theme. My intention was to clone the theme and change the information on the form to match my needs.

Bummer....I was hoping I could use the theme.
