The spreadsheet was sorted alphabetically by last name. The header row was included in the sort. Could this have caused a problem?

Can you please tell us the form ID/URL that have the issue? I checked your username SARJKing but I found that you do not have form/s under the username.

Sorting the column header in the google spreadsheet side will not break the existing integration. So it is fine to do that.
The issue could be due to a different reason. So we need to identify which form it is so we can do further test and check.

I cloned your form for testing purposes and I can't seem to replicate the issue.
I have integrated google spread sheet in my cloned form and I was able to receive my submissions.
Here is a sample screen shot.
Kindly reintegrate Google Spread sheet on your form and let us know if the issue still remains.

I don't understand what you mean by reintegrate my Google spreadsheet. Please explain the steps to do this. Thank you.

Please be noted that new integration will create a new Spreadsheet (it will not keep the old Spreadsheet file).
