Why is my account suspended?

I have checked your account and it was suspended due to the form http://www.jotform.us/form/80434944984165. It's an loan application form and you're asking for personal information, but your account email address is under a free email provider, please provide us with a proof of your business and a corporate email address so we can reactivate the account. 

We may also delete the form from your account in order to reactivate it.

I have just reactivated your account, please go ahead and change your email address to the corporate/company email.

Also, we highly recommend adding a sort of agreement or an e-signature field to the form for your clients to confirm that they have agreed to provide their personal information.
