Write in text on a radio field.

    Hi,

    In my form I have a radio field:

    Hello Is your day good?

    0 - Yes

    0 - No

    0 - Other_______________________________________

     

    How can I have it so when someone clicks 'other', they can input some text?

    I have a field that needs 2 spots to write in on 1 line.

    Year and Months.

    Is this possible?

     

    Example of how a fishied question should look:

    How long have you lived here:

    (2) years          (5) motnhs

    Is it possible to make it so the date & time cannot be changed by the user?

    @coralsprings954,

    Unfortunately, it is not possible. We don't have such option to let fields not editable.
