Is it possible to allow uploading 5 files to a form?

I am interested in your services.  I am a Professional Consultant & Organizer and I need an online form that my potential clients can submit their organization requests but it must have the capacity to download up to 5 photos.  Do any of your templates have this feature?  Thanks in advance.

Please be noted that it is possible to create file upload forms using JotForm and each file upload field can allow uploading up to 25 files.
