Hi - i have a patient satisfaction survey form which has 30 physicians listed in a dropdown menu and have made conditional rules for the form to be sent to 1 of 8 different emails depending upon who the patient went to. the name and email is optional in this form.
Is there a way to not have it say the sender is jotform without puting in a specific email? Because there is no "master" email - the form will be sent to the specific practice managers....
i can add that the sender email is from the person filling it out....but it is not a required field
and the reply-to address depends upon the practice that it was sent to.

My suggestion is to use JotForm's sender email which is "email@example.com" (This is to increase the delivery rate of every notification email being able to reach its destination and to avoid notification emails being caught by your mail spam filter) then on the Reply-to address you can select the form fillers e-mail which is called "E-mail (optional)" according to your form. This way, when you reply to your form user's submissions, all "Reply-to" address will be prefilled with your form user's email address. Check this screen:

Unfortunately, there's no other way than this. JotForm's default sender email address (firstname.lastname@example.org) will be automatically used when Sender's Email or Reply-to Email is left empty.
