One of my survey questions is not showing up on the notification report

One of my survey questions is not showing up on the notification report, but does show up on the spreadsheet.  I want it on the initial report also.

I did check your form's notification email alert and found that they are edited. Please note that if your email alert template is edited, changes made to your form will not reflect in your email alert automatically. 

The best way to solve this issue is to delete your notification email alert and add a new one. A new email alert will include all the questions you have in your form. The following guides should help you:
