Unable to open form, error 'This form is currently unavailable!'

I've just upgrade to Maxim Plan, and the form is unavialable.

Why?

The form is currently disabled. Please enable it, here's how:

In My Forms page, select the form and then click the "More" button. After that, select the "Enable" option.
