jot form not opening in safari

I have a client trying open a form on safari and it is not opening.

I tried on safari and am having the same problem.  I'm getting a blank screen - same issue that he is having.  Screen shot attached

I cleared your form caches and the form appears to be loading once again.  Check to see if it is on your end.  If this does ever happen again, try clearing your form caches and it should resolve the issue:
