How can we add IP Address to our email notification

I added the IP Address to the submission form.  When I say send a test email it comes through with the IP Address, but when I submit an actual submission the IP address is not there.

Also, I want the "From" to be labeled as a "New Application."  When I say send a test email it comes through with the correct "New Application", but when I submit an actual submission the from field in the email is a long random set of letters/characters.

I have cloned your form and I could not replicate the issue. I was able to receive a notification with the IP field.

I have subsequently cleared your form cache, kindly do test and let us know if the issue is resolved.

Can you please go to your submissions page. Click on the Settings icon on the right. And enable the IP option.

Please let us know if the issue persists. Thank you!

Apologies for the challenges you are facing.

Would you kindly delete the notification you have setup then create a new notification with the IP fields added then test new notification and let us know if this works?

I can see that your form is encrypted, the email notification for encrypted forms will have the default data and it cannot be customized, email autoresponders are not available on encrypted forms either.
