My Form is not showing up on website

My form link is not showing visible on my website and I cannot log into my account.

I have checked your account and see that it is suspended due to the form http://www.jotform.us/form/50695661099164 which is collecting Twitter password. Please note that it is not allowed to create login forms, collecting sensitive information such as passwords, credit card information using JotForms. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please go through our terms of usage page for more information on phishing. I have now removed the password field from the form and activated your account. You should be able to view the forms without any issue.
