As you have mentioned that you already have a form at Jotform, I can also recommend to switch it's layout to see how it would be.
It is very easy to change the layout of your existing form to CARDs.
Please go to your form's Settings > click Show More Options > scroll down a bit to find Form Layout section > change the layout to Single Question in One Page.
