I recently had a customer fill out one of my forms and went to submit payment and it came back saying it was unsuccessful and that they were not charged. Form still submitted but payment did not. My Stripe account was connected successfully so I am trying to determine where the issue is at with this form.

I have tried submitting your JotForm and was able to submit the form without entering the payment information. This is due to the payment field is not set as a required field. I was able to remove the checked item from the cart and submit the form. Please make the payment field as a required field so that the user will not be able to submit the form without entering the credit card information.
