Suspended account ?

    Hello. I have 65 applications that I've spent a lot of money to generate on your platform. I just went to login and my account is suspended. If I need to make a payment that is fine, but that option was not provided.

    Please advise.

    Your account was suspended due to violation of JotForm's Terms-of-Use, because of your form ID80504918284157 "Client Information".

    It has several fields collecting respondent's sensitive information, like "Social Security #", "Drivers License #", requesting login credentials.

    Collecting Sensitive Information. You may not use the JotForm Service to collect certain types of sensitive information, including but not limited to, credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm, Inc. including SSL and Encrypted Forms features.


    In order to reactivate your account, we will need to delete those offending fields and receive a confirmation from you that you have read JotForm's Terms-of-Use and agreed.

    Thank you for your quick response. 

    That's fine. That form can be deleted entirely. It was only a secondary form for after a consultation with a new client. 

    They only receive it after a recorded phone call asking for our help. That said, I'll happily delete that form and collect the information in another format.

You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

    Please reinstate my account and I will remove that form completely.

    I have reactivated your account, but I have also deleted some fields mentioned above. 

    Please try to log in and check your other forms not to be autosuspended.
