Can't access account

    Hi there. I am working with Gwen at Topeka LASIK Center in Topeka, KS. We are currently updating their website & are trying to access Gwen's JotForm account. For the life of us, we can't figure out the username or email address associated with the account so we can change the password. Gwen receives notifications of persons filling out these forms at she also feels like the emails might be routed from, but like i said we've tried both of these and more in the "Forgot Password" section of JotForms. No luck.

    Please let us know if there if there is anything we can do to access our account. I look forward to hearing back from you. Thanks for your time.
