Some fields will not show up on the PDF attachment

    Some of the form fields do not show up on the PDF attachment with the notification email. One of these fields is required so it is not empty upon submission. Even if it were empty, it wouldn't matter because the empty fields are set to print. The other two fields are read-only, as they are meant for internal use after the form is printed. None are hidden.

    These fields do not show up on PDF:

    • My household includes residents in these age groups:


    • MCHS Use Only

    • Application Approved

    Please let me know what I need to do to correct this issue.

    To restore a default PDF layout please delete a PDF Design report under a Reports tab.
