Why is the configurable list not loading properly?

    In the first link you sent over all of the items
    in the list are not present. The link below has that issue corrected. I
    am not sure what the issue was but this one seems to be correct. Take a
    look and see if you can see what I'm talking about

    This is because of the number of fields the widget has, the widget can be configured to load  properly by re-positioning the  fields with some CSS code, please check this guide for more  details: https://www.jotform.com/help/456-How-to-Make-the-Configurable-List-Widget-Mobile-Responsive
