I need to be able to style the buttons just like the example form. Can I do this on jotform? if so, how can I accomplish this. Thanks.

I did not see any form that has a radio button on the website you shared with us. However, I see this:
If you are going to create a form with the traditional layout, I believe that would be possible with the help of CSS codes that can be injected into the form. You can also style the radio button field from the Advanced Designer page — Form-Designer-Tutorial-Let-s-create-fantastic-forms-
However, with JotForm Cards (new layout), like what you are using in all of the forms in your account, then I'm afraid it would not be possible.

Thank you for the clarification, I appreciate it. However, I'm afraid we do not have something that's exactly similar to the form on the website you shared with us.
But perhaps you may find the Image Choice field interesting.
You can upload the images to the options or you can select from the available icons.
