404 error when accessing form from link

We are having a problem with a link we are sending through jot form.  some people are receiving a "404 error" message when they hit the prompt.  do you have a suggestion.

I checked the form that is linked in your page and it is loading without issue on my end.  Is the error showing when you access the form?  If so let us know how we can reproduce the error and our end and we will be happy to have another look.  As far as I can tell though, the form is fully functional.
