It seems you have filled some other form and and auto-responder email was sent to you. That is the reason why you have received email from form.
Upon your request, I have gone ahead and unsubscribed you from our list. Please note that you will still receive email if you fill up any form and auto-responder email is sent to you.
You have also created a free JotForm account as well. Do get back to us if you want us to delete your account as well, we will do the needful
