How to Print off a survey for people who do not have the capacity to fill in online?

I would like to print off a survey for people who do not have the capacity to fill in online.

I can not get the "other" sections to be blank and the form looks disjointed- even when saved as a PDF

After that, you can use it for offline to collect data. Save filled PDFs as separate files with Adobe Acrobat Reader or Foxit Reader, so you will be able to submit them later when you're online.
