    Dear colleagues of Jotform,


    my account has been suspended. Probably this is because you found that in the form were requested sensitive documents (passports of people). I consulted in the past with you concerning this issue and you suggested to request in the form the signature of the person and add in the form the widget "terms and reference". 

    In case this is not enough, could you tell me how the situation can be solved? The purpose of requesting such document is for academic reasons (academic exchanges).

    Yes you were suspended because form 72332503716349 Erasmus+ Teaching Mobility was found to be collecting sensitive passport information.

    I understand that you were told to add a terms widget plus a signature field. However, they are both not present on this form.

    In order to continue you must agree to add the terms widget and a signature field to your form. And any other forms with sensitive information in future.

    We will wait for confirmation from you. Then we will reactivate you account so that you can make the modifications necessary to your form.

    OK, as agreed I have reactivated your account. And whitelisted your form so this doesn't happen again (with the same form).

    Please add those fields to your form.
