Account suspended

    hello. I am a landlord trying to use jotform for a rental application. I had a friend that had already built his own jotform app and offered to modify his for me to use. His name is Corey Jacobs. I changed my password and gave him access last night to set up my form. It worked fine this morning and now I’m seeing that you have suspended my account and that the form is not working. I’m not trying to do anything shady...I would imagine that people pay virtual assistants to create forms and use their account for that often. 

    Please advise ice what I need to do to get this fixed.

    I have checked your accoutn and see that it is suspended due to the form which is collecting SSN numbers, Please note that it is not allowed to collect sensitive information such as credit card information, user login forms, etc using JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please check our terms of usage for more information on phishing forms. However, you may be able to collect SSN for certain forms and business type. Please see the following section provided below from Terms of usage.

    Collecting Sensitive Information. You may not use the JotForm Service to collect certain types of sensitive information, including but not limited to, credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm, Inc. including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

    Please refer to the guide below that can hlep you with more information on encrypted forms.

    We have now activated your account and you should be able to access the forms normally. You may enable the Form encryption on the form or delete the SSN field from the form immediately to avoid any further suspension on your account.

    I was just able to log in. We have a newborn so I didn’t have time to get on earlier. The form isn’t there now at all. Can you restore it to “my forms” so I can modify it?

    When collecting SSN information, you must comply with the following.

    1. Add a Terms of use widget (for your users to read and agree) in your form. This will surely make your forms not to be considered as guilty of phishing activities

    2. Add a Signature widget where your users sign and agree to provide such sensitive information.

    Please be aware that online form builders, including JotForm, are being used for identity theft in the past.

    We go to great lengths to prevent this. That is why SSNs are considered as a Phishing activity as stated in our Terms.

    Therefore, when you do request SSN, our automated phishing detector will flag the form(s)  and suspend the account or this will be manually performed by our Staff, if your account is on paid status, you should get a notification about it.
