Is the pricing for receiving payments based on forms example 5 per form or per subscription?

Please note that in free account you can only receive up to 3 payment submissions per month. That means all your forms put together can receive up to 3 payment submissions in a month. If you reach payment submission limit, all your forms which has payment tool will be disabled. To enable forms again, you will have to upgrade your account or wait for the first day of next month when the submission counters are reset.
