I cannot get Trello linked to account

I have created a form and linked the form to my Trello account but cannot link from Trello to jot form.  The form creates a card in Trello but it is blank

I have checked the form in your account and I can see you have mapped one field, so far, if the full name field has data the Trello card should have data as well. 

May you please check this and confirm that the data is sent? 

You may also try deleting the integration and adding it again, re authenticating should help to clear any possible issue with the current integration.
