Is it secure to use JotForm to collect medical information?

I have a medical form set up and I was told to check out and make sure it is legally binding/credible/acceptable and of course, secured. I am asking clients to agree to certain things and disclose medical information, as I need to be sure that I am protected in case something were to happen and a client tried to sue me along these lines.

I am just relating from another source. I hope you you understand what I am trying to say.
