Form loading error

I checked your form and I have replicated the issue.

Checked your fields, and you have fields that are hidden but are required. This will cause some errors in your condition.

To fix the issue, I had to unrequire the fields that are hidden.

By Holding on the CTRL key then pressing  another field,it will trigger a help pop-up option which we can select all fields and unrequire each fields.

Also, I had to disable all your conditions, it was repetitive and one of the major reason your form was having an error in starting up.

After removing all the conditions  and unrequiring all the hidden fields, your form can be loaded successfully.
