I am doing some further investigation for our team on Salesforce integration. From what I can see it appears that the integration is only setup for Accounts, Contacts , and Leads but not CASES. Is this correct? The initial response below seems to indicate it can be done but I am not seeing any options for Cases.

Yes, all fields should be mapped using the salesforce integration.

That is correct. For now our salesforce integration can only send data to Leads, Contact and Accounts and currently doesn't support CASES.
However, you can create a new thread for Feature Request so we can escalate it to our Development team.
For now, the alternative way is to use Zapier in order to integrate salesforce using CASES. https://zapier.com/apps/jotform/integrations/salesforce
