When I embed a form on my website I see my form plus part of another form

When I embed this code <script type="text/javascript" src="https://form.jotform.com/jsform/80605466384158"></script> I'm not only seeing the form I'm trying to embed but also part of another one of my forms.

When I use the iframe code, I only see my form, but I don't want to use iframe.

I'd rather use the javascript so I can edit the form to look nice on the page with out re-embedding the code all the time.
