Please check the two emails below.
Can you please explain why the email from the user is correct but the confirmation of submission from jotform is not correct.
The form questions are missing and some of the data appears multiple times
I have attached screen shots.

To fix this, please remove the existing Email Notifications and Autoresponders and create new ones.
When you create new ones, it will generate the email template with all the fields and labels.
